Ambulatory/Outpatient Medical Care
– Primary Medical Care for persons
living with HIV/AIDS is provided through Hudson River Health Care and
Institute for Family Health.
ADAP – AIDS Drug Assistance
Program is a State program that provides medications to low income
individuals who are infected with the HIV disease and have limited or no
coverage from private insurance, Medicaid or Medicare.
Early Intervention Services – Early Intervention
Services (EIS) include counseling, testing, referrals, and medical evaluations
for individuals with HIV/AIDS. EIS are being provided by AIDS Related
Community Services (ARCS).
Emergency Financial Assistance – Emergency Financial
Assistance is the provision of short-term payments to assist with
emergency expenses related to medications, food, utilities or
housing. Emergency Financial
Assistance is provided by Dutchess Outreach.
Medical Case Management/Treatment
Adherence – Medical Case Management services provide a link between
clients and HIV primary care, psychosocial and other supportive
services. Medical Case Management
services are provided by ARCS and Catharine Street
Community Center. Treatment Adherence services offer
supervised peer-based medication adherence and support. Treatment Adherence services are
provided by Hudson River HealthCare.
Home Delivered Meals Services – Home Delivered Meals
provides meals for everyday of the week to persons homebound due to
HIV-related illness. This service
is made available through the Giving Individuals Food For the Soul (GIFFTS)
Program, housed at Dutchess Outreach.
Mental Health Services – Mental Health services
provide psychological and/or psychiatric treatment and counseling to
individuals diagnosed with mental illness throughout Dutchess County.
This is service is provided by the
St. Francis Hospital Ryan White Program.
Substance Abuse Services
– Substance
Abuse services provide medical or other treatment and/or counseling to assist
those with substance abuse problems.
This service is provided throughout Dutchess County
by the St. Francis Hospital Ryan White Program.
Transportation Services – Medical Transportation
services offer transportation to a HIV infected client so that he or she
may access health care services.
This service is being provided by AIDS Related Community Services
(ARCS).
